2024-2025 Education Year Fall Term PHAR 581 Advanced Community Pharmacy Practices Apprenticeship Applications
Dear Students,
Students are required to start PHAR581 Advanced Community Pharmacy Practices apprenticeship in the fall semester of 2024-2025 academic year on 07.10.2024.
- The originals of the documents should be delivered to Rümeysa YAVAŞ at the university by the end of the working hours on 27.09.2024 at the latest.
- Documents to be sent by cargo should be mailed to the school as reach on 27.09.2024 at the latest.
- Apprenticeship applications will not be accepted after 27.09.2024.
- Applications made via e-mail will not be accepted.
- To be accepted to the internship exam, it is necessary to complete the online apprenticeship form. Link and QR code are available for access.
- In order to have a hassle-free application process, it is important to apply immediately when the necessary documents are ready, without leaving the last day.
- Apprenticeship start date should be 07.10.2024 before or after this date will not be accepted.
- Any apprenticeship during the Clinical Pharmacy and Pharmaceutical Care lectures that will be started directly after this apprenticeship will not be accepted
Documents to be submitted to the faculty must be submitted in a sealed envelope that say Rümeysa YAVAŞ's attention on it.

Issues to be considered in determining the apprenticeship place:
- All trainee students must comply with the provisions of the school's apprenticeship directive.
- The responsible pharmacist of the pharmacy where the apprenticeship will be held must have at least 5 years of pharmacy pharmacist experience.
- The pharmacy where the apprenticeship will be held must be in agreement with the SGK.
- The responsible pharmacist of the pharmacy where the apprenticeship will be held and the trainee should not be relatives.
- Trainees whose pharmacies do not work on Saturdays should extend their apprenticeship period by these missing days while scheduling their apprenticeship. Likewise, the apprenticeship period should be extended for working days that coincide with official or religious holidays. The main thing is to complete 20 workdays.
- It is mandatory to comply with attendance and institution working hours in all apprenticeship.
Documents to be submitted for apprenticeship application;
- Apprenticeship application forms signed by the trainee and the responsible pharmacist of the phamacy accepting the trainee. (with a start and end date specified). (3 copy) Link
- 1 photocopies of identity cards
- A signed petition stating that the trainee wants to do the apprenticeship voluntarily. (1 copy) Link
- Declaration of Healthcare Coverage form filled and signed by the trainee. (1 copy) Link
- Social Security Registration from filled and signed by the trainee. (1 copy) Link
